Custody and Child Support Intake Form

Need custody, parenting time or child support paperwork in Clark County? This intake covers new custody cases for parents who were never married, plus modifications and paternity. It takes about 15 minutes.

Before You Start

  • Have each child’s full name, date of birth and every state they lived in during the last 5 years.
  • Know each parent’s employer and gross monthly income, if possible.
  • Clark County generally requires both parents to complete the COPE parenting class in custody cases.
  • Do not enter Social Security numbers or full account numbers. Those are collected securely when your documents are prepared.

What Happens Next

  • I review your intake and confirm your quote in writing.
  • I prepare your documents at your direction, based on the information you provide.
  • You review and sign. In-person and mobile appointments are available anywhere in Clark County, and remote online notarization is available nationwide.
Through marriage, a signed Voluntary Acknowledgment of Paternity, or a court order.
This helps make sure the right forms are used. You may discuss it by phone instead.
