Divorce Intake Form

Ready to start your uncontested divorce? This intake form collects the information needed for a Nevada Joint Petition for Summary Decree of Divorce, based on the forms published by the Nevada courts’ family law self-help resources. It takes about 15 to 20 minutes. Once you submit it, I’ll review your answers and contact you with next steps and a quote.

Before You Start

  • Both spouses should agree on every term: custody, parenting time, child support, property, debts and alimony.
  • At least one spouse must have lived in Nevada for 6 weeks, and you’ll need a Resident Witness who can confirm it.
  • Have handy: your marriage date and place, each spouse’s employer and gross monthly income, and a list of property and debts.
  • Do not enter Social Security numbers or full account numbers. Those are collected securely when your documents are prepared.

What Happens Next

  • I review your intake and confirm your quote in writing.
  • I prepare your petition, decree and supporting forms at your direction, including the Civil Cover Sheet, Confidential Family Court Information Sheet and General Financial Disclosure Form.
  • You both review and sign. Mobile, in-person and remote online notarization are available.
  • You receive filing instructions for the Eighth Judicial District Court, Family Division.
