Estate Planning Intake Form

Planning a will, trust or powers of attorney? This intake gathers what is needed to prepare Nevada estate planning documents at your direction. It takes about 10 to 15 minutes.

Before You Start

  • Have the full names of your children, beneficiaries, executor or trustee, and agents.
  • Know which assets you own in general terms. Account numbers are not needed.
  • Think about who you would trust to make financial and healthcare decisions if you could not.
  • Do not enter Social Security numbers or full account numbers. Those are collected securely when your documents are prepared.

What Happens Next

  • I review your intake and confirm your quote in writing.
  • I prepare your documents at your direction, based on the information you provide.
  • You review and sign. In-person and mobile appointments are available anywhere in Clark County, and remote online notarization is available nationwide.
Include children from prior relationships and any deceased children who left children of their own.
Names, relationship, and shares or specific gifts.
